Acknowledgment of Risk

I, the undersigned, understand and acknowledge that operating or riding as a passenger on a jet ski involves inherent risks and dangers, including but not limited to: drowning, collision with objects, falls, personal injury, or even death. I understand that these risks may result from my own actions, the actions of others, weather or water conditions, or equipment failure.

Assumption of Risk

I voluntarily choose to participate in the jet ski activity provided by Queensway Laser Clinic and accept all risks of injury, harm, or damage that may arise from this activity.

Release of Liability

In consideration for being permitted to participate, I hereby release, waive, and discharge Queensway Laser Clinic, its owners, employees, affiliates, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) that may be sustained by me during or as a result of participating in this activity.

Medical Fitness

I certify that I am physically fit to operate or ride as a passenger on a jet ski and that I have no medical condition which would prevent me from safely participating.

Indemnification

I agree to indemnify and hold harmless Queensway Laser Clinic from any loss, liability, damage, or costs (including legal fees) they may incur due to my participation in this activity.

Consent to Rules

I agree to follow all instructions provided by Queensway Laser Clinic staff and/or the jet ski operator regarding safety and conduct during the activity.

Acknowledgment

I have carefully read and fully understand this waiver and release of liability. I voluntarily sign it, intending to be legally bound.
